Atropine Methonitrate   For treatment of congenital hypertrophic pyloric stenosis in infants, 200 to 600 mcg half an hour before feeds.  
Atropine Sulphate 400 to 600 mcg four to six times a day sc, im, or iv; as antidote to cholinesterase inhibitors by iv inj. 2 to 4 mg initially, followed by im inj. 2 mg repeated every 5 to 10 minutes. Anticholinergic,orally.250mcg to 2mg daily in single or divided doses 400 to 600 mcg four to six times a day sc, im, or iv; as antidote to cholinesterase inhibitors by iv inj. 2 to 4 mg initially, followed by im inj. 2 mg repeated every 5 to 10 minutes.

Benzylpenicillin Sodium     The equivalent of 1.2 to 2.4 gm of benzylpenicillin daily, in 4 divided doses by im or by slow iv or by infusion.
Betamethasone   0.5 to05 mg daily in divided doses.  
Betamethasone Sod. Phosphate     The equivalent of 0.5 to 5 mg of Betamethasone daily, in divided doses. Acute adrenal insufficiency, by iv or im inj., the equivalent of 10 to 80 mg of Betamethasone daily in divided doses.
Bisacodyl   5 to 10 mg daily.  
Bismuth Subcarbonate   1 to 4 gm.  
Bleomycin Sulphate     By inj. The equivalent of 15 to 30 Units of Bleomycin weekly in divided doses.
Bromhexane HCl.   8 to 16 mg three to four times daily.  
Bromocriptine Mesylate   The equivalent of 2.5 to 20 mg of Bromocriptine daily in divided doses.  
Bupivacaine HCl     By infiltration anaesthesia, the equivalent of 2 mg of anhydrous Bupivacaine Hcl per Kg body weight.
Buprenorphine HCl   Sublingually, upto 400 mcg every 6 to 8 hrs. 300 to 600 mcg every 6 to 8 hrs;im or slow iv;
Busulphan 2 to 4 mg daily Maintenance dose, 0.5 to 2 mg daily.  
Caffeine   300 to 600 mg  
Calcium Carbonate   1 to 5 gm.  
Calcium Chloride   1 to 2 gm 5 to 10 ml of 10 % w/v solution by slow iv
Calcium Folinate     Upto 120 mg in divided doses over 12 to 24 hrs by im or iv inj. Or infusion followed by 12 to 15 mg im or 15 mg orally every 6 hrs for the next 48 hrs.
Calcium Gluconate   Upto 15 gm daily, in divided doses 1 to 2 gm by im or slow iv Inj.
Calcium Lactate   Upto 8 gm daily, in divided doses.  
Calcium Levulinate     1 gm once a day, by im or iv. Inj.
Calcium Pantothenate   10 to 100 mg daily, in divided doses  
Captopril 12.5 to 50 mg twice daily Usual maintenance dose, 25 mg twice daily, maximum, 50 mg twice daily.  
Carbamazepine 200 mg daily Increasing to 1.2 gm daily, in divided doses, in accordance with the needs of the patient.  
Carbenicillin Sodium     By iv inj., the equivalent of 12 to 30 gm of Carbenicillin daily, in divided doses; by im inj., the equivalent of 4 to 8 gm of Carbenicillin daily, in divided doses.
Carbenoxolone Sodium   300 mg daily, in divided doses for 1 week; subsequently, upto 150 mg daily, in divided doses.  
Carbidopa   10 to 25 mg in combination with Levadopa  
Carbimazole Controlling dose, 30 to 60 mg daily, in divided doses Maintenance dose, 5 to 20 mg daily.  
Castor Oil   5 to 15 ml.  
Cefadroxil   500 mg to 2 gm daily, in divided doses.  
Cefazolin Sodium     1 to 4 gm daily in divided doses, by im or iv or infusion.
Cefotaxime Sodium     “1 to 2 gm of Cefotaxime Every 8 to 12 hrs by im or iv Inj.; By infusion, 1 to 2 gm given over 20 to 60 minutes.
For children, the equivalent of 100 to 150 mg of Cefotaxime per kg body weight daily in 2 to 4 divided doses increasing to 200 mg per kg of body weight daily, if necessary.”
Ceftazidime     1 to 2 gm, every 8 to 12 hrs, by im or slow iv or iv infusion.
Cefuroxime Sodium   250 mg twice daily 750 to 1.5 gm, every 6 to 8 hrs, by im or iv or iv infusion.
Cephalexin   1 to 4 gm daily, in divided doses.  
Cephlorididine     1 to 4 gm daily, in divided doses, by iv, im or deep sc inj.
Chlorambucil   100 to 200 mcg per kg of body weight daily for 4 to 8 weeks.  
Chloramphenicol   Adult, 1.5 to 3 gm daily, in divided doses; for a child, 25 to 50 mg per kg of body weight daily, in divided doses.  
Chloramphenicol Palmitate   Adult, the equivalent of 1.5 to 3 gm of Chloramphenicol daily, in divided doses; for a child, the equivalent of 25 to 50 mg of Chloramphenicol per kg of body weight daily, in divided doses.  
Chloramphenicol Sodium Succinate     The equivalent of 3 to 4 gm of Chloramphenicol daily, in divided doses, by iv inj.
Chlorcalciferol   “Rickets; not more than 800 units daily.
For the treatment of rickets &osteomalacia, 5000 to 50000 Units daily
For the treatment of hypocalcaemia andhypoparathyroidism, 50000 to 200000 Units daily.”
200000 to 400000 Units by im inj.
Chlordiazepoxide   10 to 100 mg daily, in divided doses.  
Chloroquine Phosphate   “Prophylactic,300 mg once weekly; therapeutic,initial dose, 600 mg followed by a single dose of 300 mg after 6 to 8 hrs and then by a single dose of 300 mg daily for the next 2 to 4 days & in
hepatic amoebiasis 600 mg daily for 5 to 7 days followed by 300 mg to 450 mg daily or twice a week for a week for 2 weeks or longer
By im inj. 160 to 200 mg repeated intervals of 12 hrs until oral therapy is possible as Antimalarial and in hepatic amoebiasis
Chloroquine Sulphate   Prophylactic,300 mg once weekly; therapeutic,initial dose, 600 mg followed by a single dose of 300 mg after 6 to 8 hrs and then by a single dose of 300 mg daily for the next 2 to 4 days & in
hepatic amoebiasis 600 mg daily for 5 to 7 days folloved by 300 mg to 450 mg daily or twice a week for a week for 2 weeks or longer
“By im inj. 160 to 200 mg repeated intervals of 12 hrs until oral therapy is possible as Antimalarial and in hepatic amoebiasis
Chlorpheniramine Maleate   4 to 16 mg daily, in divided doses 10 to 20 mg, by sc or im inj. Repeated if required; maximum 40 mg in 24 hrs. By slow iv inj. over 1 minute, 10 to 20 mg diluted in the syringe with 5 to 10 ml of blood.
Chlorpromazine HCl   As antipsychotic, 75 to 300 mg daily, in divided doses; As antiemetic, 10 to 25 mg every 4 to 6 hrs. As antipsychotic, by im inj. 25 to 50 mg. As antiemetic, by deep im inj. 25 mg initially followed by 25 to 50 mg every 3 to 4 hrs until necessary.
Chlorpropamide   100 to 500 mg daily.  
Chlorthalidone   50 to 200 mg daily.  
Cholera Vaccine     “For adults and children over 10 years, by sc inj., initial dose, 0.5 ml; second dose, 1.0 ml after an interval of 4 to 6 weeks.
For children 2 to 10 years, initial dose 0.3 ml; second dose 0.3 ml.
For children 1 to 2 years, initial dose, 0.2 ml; second dose 0.2ml.”
Chorionic Gonadotrophin     By im inj, 500 to 5000 units twice weekly or in accordance with the needs and response of the patient.
Ciprofloxacin     100 to 200 mg twice daily by infusion.
Ciprofloxacin HCl   The equivalent of 250 to 750 mg of Ciprofloxacin twice daily.  
Cisplastin     By iv infusion, 15 to 20 mg per sq. m. of body surface daily for 5 days.
Clofazimine For leprosy, for previously untreated patients, 100 mg 3 times weekly; For sulphone- resistant patients, 100 mg 6 times weekly. For the suppression of lepra reactions, 200 mg daily.  
Clonidine HCl   50 to 100mcg thrice daily increased gradually according to the needs & response of the patient; maximum daily dose 1.2 mg. By slow iv inj., 150 to 300 mcg; maximum daily dose 750 mcg.
Cloxacillin Sodium   The equivalent of 500 mg of Cloxacillin every 6 hrs, at least 30 minutes before food; By im inj. 250 mg every 4 to 6 hrs; by slow iv inj. or by infusion, 500 mg every 4 to 6 hrs.
Codeine Phosphate   30 to 60 mg every 4 hrs when necessary, to a maximum of 200 mg daily.  
Colchicine 1 mg Subsequent doses, 500 mcg every two hrs.  
Colchicine & Probenecid Tabs   Colchicine 1 mg & Probenecid, 250 mg twice daily after food.  
Concentrated Glyceryl Trinitrate
Solution”
  Sublingually (as tablet), 300 mcg to 1 mg, repeated as required.  
Cortisone Acetate   25 to 37.5 mg daily, in divided doses By im inj. 50 to 400 mg daily, in single or divided doses.
Cyanocobalamin     For treatment of megaloblastic anaemia, by im inj. 1 to 2 mg in divided doses, in the first week. Subsequent doses, 250 mcg weekly until the blood count is normal, maintenance dose, 250 mcg every 3 or 4 weeks.
Cyclophosphamide   100 to 150 mg daily By iv inj. 100 to 150 mg daily.
Cyclserine   500 mg to 1 gm daily, in divided doses.  
Cyproheptadine HCl   4 to 20 mg daily, in divided doses.  
Cytarabine     To be determined by the physician.
Danazol   200 to 800 mg daily, in divided doses.  
Dapsone   100 mg daily.  
Dehydroemetine HCl     By deep im inj. 60 to 90 mg daily.
Desferrioxamine Mesylate   After gastric lavage, 5 to 10 gm in 50 to 100 ml of liquid; By im inj. 1 to 2 gm in 10 to 20 ml of Water for Inj. every 3 to 12 hrs; by continuous iv infusion, upto 15 mg per kg per hour with a maximum of 80 mg per kg in 24 hrs.
Dexamethasone   500 mcg to 10 mg daily, in divided doses.  
Dexamethasone Sodium Phosphate     In the treatment of adrenal insufficiency, by im or slow iv or infusion, 0.5 to 10 mg. In shock, by iv inj, or infusion, 2 to 6 mg per kg body weight, repeated if necessary after 2 to 6 hrs. In cerebral oedema, by iv inj., 10 mg initially, then 4 mg by im inj. every 6 hrs as required for 2 to 10 days.
Dextromethorphan HCl   10 to 30 mg every 4 to 8 hrs.  
Diazepam   In anxiety states, 2 mg thrice daily, increased if necessary to 15 to 30 mg daily, in divided doses; in insomnia associated with anxiety, 5 to 15 mg at bedtime By im or slow iv inj. 10 mg repeated if necessary after 4 hrs.
Diclofenac Sodium   25 to 75 mg. 25 to 75 mg.
Dicyclomine HCl   30 to 60 mg daily, in divided doses. Oral Solution is required to be diluted; the diluted solution should be freshly prepared.  
Dienoestrol   In the treatment of menopausal symptoms, 500 mcg to 5 mg daily; in the treatment of prostate carcinoma and mammary carcinoma, 15 to 30 mg daily; for suppression of lactation, 15 mg thrice daily for 3 days, followed by 15 mg daily fir 6 days.  
Diethylcarbamazine Citrate   150 to 500 mg daily.  
Digoxin “For rapid digitalisation,1to 1.5 mg in divided doses over 24 hrs; for less urgent digitalisation,250 to 500mcg daily;
For paediatric use, 10 to 35 mcg per kg body weight in 3 or 4 divided doses over 24 hrs followed by 4 to 10 mcg per kg body weight daily”

Diluted pentaerythritol Tetranitrate   The equivalent of 20 to 60 mg of pentaerythritol tetranitrate, 3 to 4 times daily.  
Dimercaprol     By im inj., 2 to 3mg per kg of body weight every 4 hrs during the first day and subsequently, in accordance with the needs of the patient.
Dimethicone   40 to 100 mg, four times daily.  
Diphenhydramine HCl   50 to 200 mg daily, in divided doses.  
Diphenoxylate HCl   5 to 30 mg daily, in divided doses.  
Disodium Edetate     By iv inj. 50 mg per kg of body weight, upto a maximum of 3 gm per day.
Disulfiram 500 mg as a single dose for 1 to 2 weeks Maintenance dose, 125 to 500 mg daily.  
Dothiepin HCl Initially 75 mg daily, in divided doses, Increased gradually, as necessary, to 150 mg daily.  
Doxepin HCl Initially, the equivalent of 75 mg of Doxipen daily,in divided doses Increased gradually to a maximum of 300 mg daily, in divided doses.  
Doxorubicin HCl     To be determined by the physician in accordance with the needs of the patient.
Doxycycline HCl The equivalent of 200 mg of Doxycycline on the first day Followed by 100 mg daily.  
D-Panthenol   250 to 500 mg.  
Dydrogesterone   10 mg twice daily.  
Emetine HCl     By sc or im inj. 30 to 60 mg daily.
Enalapril Maleate Initial, 2.5 to 5 mg daily Maintenance dose, 10 to 20 mg daily; maximum 40 mg daily.  
Ephedrine   15 to 60 mg.  
Ergocalciferol   “Rickets; not more than 800 units daily.

Ethinyloestradiol   “For menopausal symptoms, 10 to 20 mcg daily continuously or for 21 days, repeated after 7 days, with a progestogen from day 17 to day 26 of cycle if uterus is intact.
For primary amenorrhoea, 10 mcg on alternate days increasing to a maximum of 50 mcg daily continuously with a progestogen for the last 5 days of month.”

Ferrous Gluconate   Prophylactic, 600 mg daily; therapeutic, 1.2 to 1.8 gm daily, in divided doses.  
Ferrous Sulphate   Prophylactic, 300 mg; therapeutic 600 to 900 mg daily in divided doses.  
Fludrocortisone Acetate   In adrenocortcal insufficiency, 50 to 300 mcg daily.  
Fluphenazine Decanoate     By deep im inj. test dose 6.25 to 12.5 mg, thereafter 4 to 7 days 12.5 to 100 mg repeated as required.
Fluphenazine HCl   1 to 2 mg twice daily, in anxiety states; 2.5 to 10 mg daily in 2 to 3 divided doses in schizophrenia & other psychoses By im inj. 1.25 to 10 mg daily.
Flurbiprophen   150 to 200 mg, daily in divided doses, increased in acute conditions to 300 mg daily.  
Folic Acid   “In the treatment of megaloblastic anaemia associated with folic acid deficiency, 5 to 20 mg daily.
In the prophylaxis of megaloblastic anaemia of , 200 to 500 mcg daily.”
 
Fortified Benzathine Penicillin Inj.     To be determined by the Physician in accordance with the needs of the patient.
Frusemide   In oedema, 20 to 40 mg daily; in oliguvria, 250 mg 4 to 6 times daily; By im or slow iv inj. 20 to 50 mg
Furazolidone   400 mg daily, in divided doses.  
Fusidic Acid   500 mg to 1 gm every 8 hrs.  
Gentamicin Sulphate     By im or slow iv inj. over at least 3 minutes or by iv infusion, 2 to 5 mg per kg body weight daily, in divided doses.
Glibenclamide   5 mg daily, adjusted according to response; maximum 15 mg daily, after food.  
Griseofulvin   500 mg to 1 gm daily, in divide doses.  
Guaiphenesin   “Adult: 200 to 400 mg every four hrs;
Child aged: 6 to 12 yrs, 100 to 200 mg every four hrs;
Child: 2 to 6 yrs, 50 to 100 mg every four hrs.”
Gugulipid   500 mg, equivalent to 25 mg of gugulsterones, thrice daily.  
Haloperidol   1.5 to 20 mg daily, in divided doses; By im or iv inj. 2 to 10 mg.
Heavy Magnesium Carbonate   “As antacid; 300 to 600 mg;
As laxative, 2 to 4 gm.”

Heparin Sodium     Prophylactic, by sc inj. 10000 to 15000 Units daily, in divided doses; therapeutic, by iv inj. 20000 to 50000 Units daily, in divided doses.
Hyaluronidase     By sc or im, inj. 1500 Units either mixed with the inj. fluid or injected into the site before inj. is administered.
Hydralazine HCl   25 to 50 mg twice daily; By slow iv inj. 5 to 20 mg over 20 minutes; may be repeated after about 30 minutes.
Hydrochlorothiazide   25 to 100 mg.  
Hydrocortisone   Adrenocortical insufficiency, 20 to 30 mg daily, in divided doses; By im inj. or by slow iv inj. or infusion, 100 to 500 mg, 3 to 4 times in 24 hrs or as required.
Hydrocortisone Acetate     By intra- articular inj. or local infiltration, 5 to 50 mg.
Hydrocortisone Hemisuccinate     By iv inj., the equivalent of 100 to 500 mg of Hydrocortisone 3 to 4 times in 24 hrs or as required.
Hydroxocobalamin     In the treatment of megaloblastic anaemia, by im inj. 1 to 2 mg in divided doses, in the first week; subsequent doses, 250 mcg weekly until the blood count is normal; maintenance dose, 1 mg every two months.
Hydroxyprogesterone Hexanoate     By im inj. 250 to 500 mg weekly during first half of .
Hyoscine Butylbromide   20 mg four times daily; By im or iv inj. 20 mg, repeated after 30 minutes if necessary.
Hyoscine HBr   In the prevention of motion sickness, 300 mcg 30 minutes before commencing journey and repeated every 6 hrs; If required; by sc or im inj. for pre anesthetic medication, 200 to 600 mcg.
Ibuprofen   600 mg to 1.2 gm daily, in divided doses, after food.  
Imipramine HCl.   50 to 150 mg daily, in divided doses.  
Indomethacin   50 to 200 mg daily, in divided doses, with food  
Isoniazid   300 mg daily or upto 1 gm twice weekly.  
Isoprenaline HCl.     By iv infusion, 0.5 to 2 mcg per minute, usually administered after dilution with Dextrose Inj.
Isoprenaline Sulphate   5 to 20 mg daily.  
Isoxuprine HCl.   As vasodilator, 20 mg 4 times daily; “As vasodilator ,by im inj. 5 to 10 mg upto 4 times daily;
By infusion, initially 100 mcg per minute, increased to 500 mcg per minute if required
As uterine relaxant, by iv infusion, initially 200 mcg to 300 mcg per minute gradually increased to 500 mcg per minute until labor is arrested; subsequently by im inj 10 mg every 3 hrs for 24 hrs, then every 4 to 6 hrs for 48 hrs; then orally, 20 mg 4 times daily.”
Ispaghula Husk   3 to 5 gm.  
Kanamycin     “By im inj. the equivalent of 500 mg to 1 gm of Kanamycin daily, in divided doses

Ketamine HCl.     By slow iv inj. the equivalent of 1 to 2 mg of Ketamine per kg of body weight over 60 seconds, repeated according to patients response; by deep im inj. the equivalent of 4 to 10 mg of Ketamine per kg body weight, repeated according to patients response.
Ketoconazole   “Adult: 200 mg to 400 mg once daily with food for 14 days

Ketoprofen   100 to 200 mg daily, in 2 to 4 divided doses, with food.  
Labetalol HCl. Initially, 100 mg twice daily with food; Increased at fortnightly intervals to usual dose of 200 mg twice daily; upto 88 mg daily in divided doses; maximum 2.4 gm daily; By iv inj. 50 mg over 1 minute, repeated after 5 minutes, if necessary; maximum 200mg.
Levamisole HCl   The equivalent of 150 mg of Levamisole as a single dose.  
Levodopa Initially, 125 to 500 mg daily, in divided doses after meals, Increasing gradually in accordance with the needs of the patient; optimal dose, 1 to 8 gm daily. Usually used in combination with Carbidopa.  
Levonorgestrel   As a contraceptive, 150 to 250 mcg in combination with 20 to 50 mcg of ethinyloestradiol daily.  
Light Kaolin   15 to 75 gm.  
Light Magnesium Carbonate   “As antacid; 300 to 600 mg;
